Virtual consultations with specialist

Virtual consultation with a specialist

If you are undergoing a medical procedure for diagnostic purposes or, haven been diagnosed with a condition covered by the benefit, and provide us with: a referral to a specialist and/or other medical document* indicating the need for an examination for the presence of a covered condition, you will be entitled to up to two virtual consultations per year for each medical specialty with selected specialists both national and international*.

  • Cancer - all malignant tumours, tumours in situ and precancerous lesions
  • Cardiovascular diseases - coronary heart disease, heart failure, cardiac arrhythmias
  • Neurosurgery - all intracranial conditions (intracranial surgery) and benign tumours of the spine
  • Transplantation - organ transplant from a living donor, including kidney, liver fragment, pancreas or lung lobe, bone marrow transplant

Patient Navigation

Once you have been diagnosed with a covered condition and have provided medical documentation confirming the diagnosis or including a recommendation for a covered surgical procedure, you will be entitled to 2 virtual consultations with a Patient Navigator for each medical specialty. You will also receive a report including guidance in the following areas:

  • Condition and treatment
  • Interaction with Healthcare Team
  • Patient Organizations
  • Activities of daily life / Self-care
  • Financial support

Treatment Abroad

In the event of receiving a diagnosis of one of medical conditions or a recommendation for a medical procedure covered by the benefit, we will arrange treatment for you at one of foreign facilities. In addition to covering the cost of treatment, you will have access to the following benefits* as part of this service:

  • Coverage of travel and accommodation expenses
  • Interpreter support during your stay in hospital
  • Daily allowance for hospitalisation if you stay overnight in a hospital abroad
  • Coverage of the costs of medications prescribed by the attending doctor and purchased upon your return to Poland

You will be given access to your own customer product dashboard following your initial registration. Within this product dashboard you will have access to a range of services including details of the benefits available and how to access them and additional support tools. You can access the customer portal via www.

Digital Claim

Digital claim submission

To make an inquiry or request for a service, you need to register to the Trustedoctor portal. Each service in the product panel has the steps described to submit an inquiry or request online. You can claim at any time within the policy term.

  • Cancer – malignant tumours, in-situ tumours and pre-cancerous lesions
  • Cardiovacular diseases – coronary heart disease, heart failure, cardiac arrhythmias
  • Neurosurgery - all intracranial conditions (intracranial surgery) and benign tumours of the spine
  • Transplantation – Living donor organ transplantation and bone marrow transplantation

Applies to the Patient Navigation service:

  • Cancer – malignant tumours, in-situ tumours and precancerous lesions
  • Cardiovascular diseases – coronary heart disease, heart failure, cardiac arrhythmias
  • Neurosurgery - all surgical procedures on the brain or other intracranial structures as well as benign tumours of the spinal cord
  • Transplantation – living donor organ transplantation and bone marrow transplantation

Applies to Treatment Abroad service:

  • Cancer – malignant tumours, in-situ tumours and precancerous lesions
  • Cardiovascular diseases – heart valve repair or replacement procedure, coronary artery bypass procedure
  • Neurosurgery – surgical intervention of the brain or other intracranial structures, as well as benign tumours of the spinal cord
  • Transplantation – transplant of an organ from a living donor, such as a kidney, liver, lung lobe, pancreas, bone marrow transplant

Documents we require for the provision of covered services:

  1. For Cancer, the Insured will need to provide any of the following:
    • Green DILO card

    OR

    • medical record issued by a specialist doctor requesting or documenting the investigation of the presence of Cancer.
  2. For Cardiovascular diseases, the Insured will need to provide any of the following:
    • Speedy referral (cito) to cardiologist from GP or a specialist doctor triggering the investigation of the condition

    OR

    • medical record issued by a specialist doctor requesting or documenting the investigation of the condition.
  3. For Neurosurgery, the Insured will need to provide any of the following:
    • referral to neurosurgeon from GP or a specialist doctor triggering the investigation of the condition

    OR

    • medical record issued by a specialist doctor requesting or documenting the investigation of the condition.
  4. For Transplants, the Insured will need to provide any of the following:
    • referral to a transplant physician from GP or a specialist doctor, triggering the investigation of the condition

    OR

    • evidence/medical records issued by a specialist doctor requesting or documenting the investigation of the condition.

Documents we require for the provision of covered services:

  1. For Cancer, cytological or histological evidence will be required along with a diagnosis from an Oncologist.
  2. For Cardiovascular diseases, a written diagnosis from a Cardiologist stating the valid ICD code.
  3. For Neurosurgery, a written recommendation from a Neurosurgeon for the stated medical procedures.
  4. For Transplants, a written recommendation from a transplant physician for the stated medical procedures.

Documents we require for the provision of covered services:

  1. In the case of Cancer, we will ask you to provide:
    • Copy of past medical history (for breast or uterine cancer, request gynaecological past history including ultrasounds and mammogram reports). (Please note: the past medical history notes of the last 10 years starting from the effective date of the insurance policy are mandatory as per the Terms and Conditions)
    • Copy of physician/specialist notes
    • Copy of treatment plan
    • Copy of pathology reports
    • Copy of endoscopy reports (if applicable)
    • (For leukemia or lymphomas: include bone marrow aspirations and/or bone marrow biopsy reports)
    • Copy of radiology reports
    • Copy of all imaging tests performed (CT scans, MRIs, ultrasounds, ERCP, etc)
  2. In the case of cardiovascular disease, we will ask you to provide:
    • Copy of past medical history (Please note: the past medical history notes of the last 10 years starting from the effective date of the insurance policy are mandatory as per the Terms and Conditions)
    • Copy of physician/specialist notes
    • Copy of treatment plan
    • Copy of laboratory results
    • Copy of ECG tracings, 24 ECG or Holter reports
    • Copy of pathology reports
    • Copy of radiology reports
    • Copy of all imaging tests performed (CT scans, MRIs, ultrasounds, angiograms, echocardiograms, etc)
    The documentation will need to include specific reference to one of the covered diseases such as coronary heart disease, heart failure or cardiac arrhythmia, which are labelled with ICD* codes (I49, I39, I35, I08, I20-I25, I50)

    * ICD = International Classification of Diseases

  3. In the case of Neurosurgery, we will ask you to provide:
    • Copy of past medical history (Please note: the past medical history notes of the last 10 years starting from the effective date of the insurance policy are mandatory as per the Terms and Conditions)
    • Copy of physician/specialist notes (including copy of neurological assessment)
    • Copy of treatment plan
    • Copy of laboratory results (including lumber puncture and CSF analysis)
    • Copy of pathology reports
    • Copy of radiology reports
    • Copy of all imaging tests performed (CT scans, MRIs, etc)
  4. For Transplants, we will ask you to provide:
    • Copy of past medical history (Please note: the past medical history notes of the last 10 years starting from the effective date of the insurance policy are mandatory as per the Terms and Conditions)
    • Copy of physician/specialist notes
    • Copy of treatment plan
    • Copy of laboratory results
    • Copy of pathology reports
    • Copy of radiology reports
